MNSWAP 2024 Award Nomination for Volunteer of the Year

Nomination for the Volunteer of the Year needs to include no more than two letters (one page each) of support. Please do not include any identifying information such as facility name or person’s name if possible. Describe the nominee’s volunteer service, number of years/hours volunteered, uniqueness of the volunteer service, and other pertinent characteristics of the volunteer nominee. Include this form along with supporting letters and submit to the MNSWAP Vice President by August 25th.
